Anti-Inflammatory Compounds

Nonsteroidal Anti-Inflammatory Drugs (NSAIDs) are a widely used class of medications, including aspirin, ibuprofen (Advil, Motrin), and naproxen (Aleve), that reduce pain, fever, and inflammation by inhibiting COX enzymes that produce prostaglandins. Systemic NSAIDs are effective for short-term pain management but carry known risks:

  • Gastrointestinal irritation and bleeding
  • Renal stress (especially in dehydrated athletes)
  • Cardiovascular risk
  • Potential impairment of tissue healing with prolonged use

In athletic populations, repeated systemic or oral dosing during training or competition blocks can create unnecessary whole-body exposure and physiologic burden.

Topical and transdermal NSAIDs offer a more targeted approach. Transdermal ketoprofen, for example, provides potent COX inhibition with favorable tissue penetration when applied locally. For clinicians managing tendinopathies, overuse injuries, and localized joint inflammation, this delivery method allows for site-specific anti-inflammatory effects while minimizing systemic exposure.

Studiesshow that ketoprofen reduces muscle soreness and helps athletes recover muscle force faster than a placebo. Like other NSAIDs, it inhibits prostaglandin synthesis to relieve acute pain, swelling, and stiffness.

Topical ketoprofen is effective for localized soft-tissue injuries, achieving high concentrations at the injury with low systemic absorption and fewer side effects.

Choosing the Right Base for Topical Compounds

The base selected in a topical formulation does more than simply “hold” the active ingredient; it fundamentally influences how well the drug penetrates the skin, reaches underlying tissues, and ultimately achieves a therapeutic effect at the site of injury. Permeation-enhancing bases, such as Lipoderm, are specifically engineered to facilitate deeper and more efficient drug delivery compared with traditional non-enhancing vehicles. In comparative permeation studies using human skin models, ketoprofen formulated in a phospholipid-based vehicle like Lipoderm demonstrated greater overall absorption and a faster rate of penetration into and through skin than when compounded in more basic vehicles such as pluronic lecithin organogel (PLO). This suggests that the choice of base can directly influence how much active drug reaches underlying soft tissues, a critical factor for anti-inflammatory efficacy.
